It's a beautiful piano piece from the 2016 anime film "Your Name" (Kimi no Na wa, 君の名は。).
Mitsuha's Theme (三葉のテーマ, "Mitsuha no Tēma") was composed by Yojiro Noda (野田洋次郎), the lead vocalist of Japanese rock band RADWIMPS, who recorded most of the music for the film.
"Your Name" was a huge success - it was the highest grossing anime film in its release year, and the third highest-grossing anime film of all time!
The film won numerous awards, including a Los Angeles Film Critics Association Award and "Best Animated Feature Film" at the 49th Sitges Film Festival.
"Theme of Mitsuha" is a wonderful song to play on the piano!
