This is part 4 of the Delian Suite No 8 "Letters to Eternity" (cameo memorial compositions by various members of the Delian Society including this one by Joseph Dillon Ford), which was performed by the Octava Chamber Orchestra on 30 January 2016
(the sound sample comes from that performance).
Ford's original tempo marking is somewhat faster than the performance here, but both work well.
The pdf file contains the score (in Tabloid format) and parts (in Letter format).
