Good basic theory book
I like this book and the sequencing it uses to teach musical concepts. It is part of a recommended four-book set, and teaches concepts while working on execution of the concepts in the other three books.

Bastien Piano Basics, Level 1, Theory
After the Primer Level, I use this book for all my elementary age students. It focuses on fundamentals in an easy and fun to learn format.
